Erie County’ s Help Me Grow Early Intervention program is designed to assure that newborns, infants, and toddlers in Erie County have the best possible start in life. Early Intervention services are offered to children with developmental delays or medical diagnoses.

What is a Service Coordinator?

A Help Me Grow service coordinator meets with your family to gather info about your child, and conducts nutrition, vision, and hearing screenings.  They will work directly with your family to coordinate a developmental evaluation, and services, if needed.   Your service coordinator will discuss your concerns and priorities regarding your child’s development and can share information about resources in your community.

I’m concerned about my child’s development or My child has a diagnosed medical condition that is impacting their development.

What should I do?

You are in the best position to observe your child and report what your child is doing.   If your child’s development worries baby child cute doll 41307smyou, don’t wait to contact us. Talk with your health care provider about a possible referral or call Help Me Grow directly at (419) 621-3962. Our Central Intake Coordinator will assign a Service Coordinator to your family who will complete a home visit to discuss your concerns regarding your child.  If your child is eligible and shows a need for Early Intervention Services, an Individualized Family Service Plan will be developed with your family.

What is an Individualized Family Service Plan (IFSP)?

The Individualized Family Service Plan (IFSP) is a plan that you will develop with your team to address your concerns and priorities for your family and child.  Young children learn best from familiar people in familiar settings. Therefore Early Intervention Services are delivered within your family/child’s everyday routine and natural environment.  There is no cost to families for Help Me Grow Early Intervention Services.

Services

Evidence-Based Early Intervention Services

Eligible children are assigned an Early Intervention Primary Service Provider to begin visits in a child’s everyday routine and natural environment.  The Primary Service Provider coaches and models techniques to encourage development in the area of need as identified by the family.  These services build confidence and competence for the parent/caregiver so they are able to continue what they have learned and incorporate these activities into their family routine. 

 

 

 

The P.L.A.Y Project

The P.L.A.Y. Project (Play and Language for Autistic Youngsters) is an evidence-based developmental pexels photo 62272smintervention offered to families enrolled in our Early Intervention Program (birth-3).  P.L.A.Y. Project Home Consultants help parents and caregivers develop play strategies to help their child better connect, communicate and build relationships with others.  The P.L.A.Y. Project was founded in 2001 by Richard Solomon, M.D. out of Ann Arbor, Michigan and they have trained 2 of our Early Intervention Providers, Teresa McDermond and Jessica Cray as P.L.A.Y.  Project Home Consultants.  Please call (419) 621-3962 for more information about this program. 

Early Intervention Group Opportunities

Early Intervention groups provide opportunities for socialization and networking for both children and parents.  Groups are offered throughout Erie County so that children have an opportunity to experience fun, age appropriate activities.
